What to Bring to the Appointment

  • Your insurance card
  • Physician referral forms (if required by your insurance)
  • A list of current prescription medications and/or over-the-counter medications to include dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent scans (MRI/CT/X-ray) and lab reports

Insurance and Payment Information

  • Louisiana Neurologic Consultants and Dr. Charles E. Kaufman are providers for most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.
  • We request payment at the time of your appointment for services that are not covered by insurance and for insurance company co-payments.
  • If you have any questions regarding which insurance plans we accept or any billing concerns, please contact us at 225-769-7200 extension 319.
